The Momentum: Pathways to Healthcare Programme works with health professionals, patients and the public along with local health services to identify a way forward for health services in Hartlepool, Stockton and parts of Easington and Sedgefield.

• Services in or close to people’s homes
• Local clinics, where much of what was previously provided in hospital can take place; and
• a new hospital which is within easy reach of everyone living in the area.
